(Cushing, Okla.) The following information is a preliminary collision report furnished by the Department of Public Safety.

A collision occurred at 7:10 (a.m.) on April 21, 2015 on SH18 5/10 of a mile south of 56th Street (4 5/10 miles north of Cushing) Payne County.

Bejamin A. Applegate (18) of Cleveland, the driver of a 2002 Ford F150 Pickup southbound on SH 18 went left of center (for reasons unknown) striking James C. Bittle Jr. (30) of Cushing who was northbound. Bittle was driving a 2012 Ford F250 Pickup.

Applegate was pronounced dead at the scene. Bittle was transported by Cushing Ambulance to Stillwater Medical Center where he was admitted in stable condition with leg and trunk internal injuries.

One of the drivers was pinned for 1 hour and 25 minutes before being freed by the Cushing Fire Department.

Seat-belts were in use according to the DPS report and the condition of the driver is still under investigation.
