(Cushing, Okla.) The following information is a preliminary collision report furnished by the Department of Public Safety.
A collision occurred this morning at 8:11 (am) on August 20th (2015), at County Road West 68th 324 feet east of County Road South Meridian (6 miles west and 3 miles south of Stillwater) in Payne County.
Jermy Ralf Eisensmith (38) of Orlando was westbound on County Road West 68th when for some unknown reason his 2002 Dodge 1500 Pickup departed the roadway to the right. The Dodge then re-entered the roadway crossing the center of the road and then crossing the center again. Eisensmith then ran the Dodge off of the right side of the road, and re-entered the roadway again going into a broad slide. The Dodge departed the roadway a final time to the left striking a ditch, and continuing on 24 feet striking a fence and coming to a rest.
Eisensmith was pronounced dead at the scene by Medical Examiner Rory Garien with head and internal injuries.
The DPS report shows that the airbags did not deploy and that seatbelts were not in use.
The cause of the collision is still under investigation.
