(Cushing, Okla.) The following information is a preliminary collision report furnished by the Department of Public Safety.

A personal injury collision occurred on Friday, July 17th (2015) at approximately 8:15 (pm) on Norfolk Road, 3/10 of a mile north of Phillips Road, 1.7 miles south of Yale in Payne County.

Crystal Cardenas (39) of Tulsa was headed northbound in a 2003 Chevrolet Malibu while Aaron J. Alfaro (39) of Yale was going southbound in a 2000 Jeep Cherokee on Norfolk Road, the Chevrolet Malibu went left of center striking the Jeep Cherokee head on. The Jeep Cherokee then spun 180 degrees and both vehicles came to rest facing northbound on Norfolk Road. Cardenas the driver of the Chevrolet Malibu had to be extracted by Yale and Cushing Fire Departments using the Hurst tools.

Cardenas was transported by Air-Evac to Saint Francis Hospital (Tulsa) in stable condition. Alfaro and his passenger Karry Alfaro (38) of Yale were both transported by a privately owned vehicle to Hillcrest Hospital (Cushing) where they were treated and released.

The Department of Public Safety report states that the cause of the collision was going left of center. There was an odor of alcohol present.
