(Cushing, Okla.) The following information is a preliminary collision report furnished by the Department of Public Safety.
A personal injury collision occurred on September 23rd (2015) at approximately 5:20 (pm) on US64 a 0.5 miles east of County Road 36800, which is approximately 3 miles south of Cleveland, in Pawnee County.
Amber L. Ford (26) of Cleveland was driving a 2006 Honda eastbound on US64 when she crossed the center line and struck a 1956 Volkswagen that was westbound driven by Robert G. Burk (64) of Mannford. The Honda that Ford was driving departed the roadway to the left, coming to rest in a ditch and the Volkswagen departed the roadway to the right coming to a rest on the shoulder. A third vehicle, a 2004 Acura being driven by Jennifer K. Holder (42) of Cleveland that was behind the Volkswagen and was struck by debris from the initial collision. The Acura came to a controlled rest on the shoulder.
Ford was transported by Mercy EMS to St. Johns (Tulsa) where she was treated and released. Her passengers Mariah N. Bibbs (21) of Hominy, along with a juvenile also from Hominy, were transported, treated and released. Burk the driver of the Volkswagen was transported to St. Johns (Tulsa) where he was admitted in stable condition with multiple injuries. Holder was not injured.
According to the DPS report the collision was caused by inattention and or use of a cell phone.
