(Cushing, Okla.) The following information is a preliminary collision report furnished by the Department of Public Safety.
A collision occurred at about 12:10 (pm) today, Wednesday, May 27th, 2015 north of Peninsula Drive 1/10 of a mile east of US64, near Cleveland, Oklahoma.
Donald Ray Botkin (77) of Cleveland was driving a 2004 Ford F150 when it crossed the center line departing the roadway to the left for reasons unknown. Botkin then corrected to the right, going into a broad slide across the roadway, and departed the roadway to the right. The truck then went downhill striking rocks and trees, rolling one half times and landing on its top.
Botkin was transported by Mercy EMS to St Johns (Tulsa) where he was admitted in stable condition with head injuries.
DPS reports show that failure to maintain lane is the cause of the accident. The driver’s condition is still under investigation.
