(Cushing, Okla.) The following information is a preliminary collision report furnished by the Department of Public Safety.
A personal injury collision occurred January 23rd (2016) at approximately 8:31 am on SH33, at Griffith Road, 5 miles east of the city limits of Cushing, in Payne County.
The collision that involved two vehicles, and a total of seven people, occurred when a 1998 Toyota Camry failed to yield to a 2013 Dodge Durango. The Camry was northbound, and crossing the westbound lanes of SH33, and the 2013 Dodge Durango was westbound on SH33.
Elezar Lerma Vazquez (29) of Oklahoma City, the driver of the Toyota Camry, was transported by ambulance to Hillcrest Hospital Cushing in stable condition. Geovani Leon, (32), Oklahoma City, was transported by ambulance to Hillcrest Hospital Cushing in stable condition. Santos Hernandez, (25), Oklahoma City, was transported by ambulance to Saint Francis (Tulsa) in serious condition. Jacabo Ciaroj Santiago, (40), Oklahoma City, was transported by AirEvac to Saint Francis (Tulsa) in critical condition. Leon, Hernandez and Santiago were all passengers in Vazquez’s vehicle.
Tina Antoinett Crutchmer, (40), Broken Arrow, the driver of the 2013 Dodge Durango, along with a juvenile passenger of Broken Arrow, were not injured. Another passenger in Crutchmer’s vehicle, Kevin L. Crutchmer, (45), Broken Arrow, was transported by a personal vehicle to an unknown medical facility.
The Department of Public Safety report shows the cause of collision was due to failure to yield.
